A formal investigation was launched following the listeriosis outbreak that has now claimed more than a dozen lives in Canada.

CBC news reported that a day before the start of the federal election campaign, Prime Minister Stephen Harper announced that investigators should examine the factors that might have led to the outbreak, the effectiveness of federal agencies' response, and recommendations for prevention of future outbreaks.

Maple Leaf Foods CEO Michael McCain revealed two slicing machines are believed to be the most likely source of the listeria bacteria.

The outbreak was possibly an accretion of bacteria inside the slicing machines and that machineries' regular cleaning does not include disassembling them.

Thirteen people has, so far, died after contracting listeriosis from eating contaminated food products linked to a Maple Leaf plant in Toronto.
